Blood Sugar a Little High on Your Report? What Prediabetes Means and What to Do Next

The report comes back from the office health check, and one line is marked in bold. HbA1c 6.1%. Or fasting sugar 112. Nobody has said the word "diabetes", but nobody has said everything is fine either, and the festive season, with a box of sweets in every room, is not the moment most people want to think about it.

A number in that grey zone usually means prediabetes: blood sugar higher than it should be, but not yet in the diabetes range. It is very common, rarely causes symptoms, and is the stage at which changes to daily life make the most difference. It deserves one proper conversation with a doctor rather than a search at midnight.

What the numbers mean

When you eat, food is broken down into glucose, a sugar your body uses for energy. A hormone made by the pancreas moves that sugar out of the blood and into your muscles and liver. In type 2 diabetes, the body stops responding to that hormone properly, makes less of it over time, or both, and sugar builds up in the blood.

Prediabetes is the step before. The usual cut-offs, used by most laboratories and guidelines, are:

| Test | Normal | Prediabetes | Diabetes |

|---|---|---|---|

| HbA1c (average sugar over about three months) | below 5.7% | 5.7% to 6.4% | 6.5% or above |

| Fasting blood sugar (after 8 hours without food) | below 100 mg/dL | 100 to 125 mg/dL | 126 mg/dL or above |

| Two hours after a glucose drink (glucose tolerance test) | below 140 mg/dL | 140 to 199 mg/dL | 200 mg/dL or above |

Two things matter here. Diabetes is not diagnosed from a single borderline result in someone who feels well: the test is repeated on another day, or a second kind of test is done, first. And the bands are lines drawn on a continuous scale. An HbA1c of 6.3% and one of 6.5% are closer to each other than either is to 5.4%.

How common this is in India

Very. The ICMR-INDIAB study, published in The Lancet Diabetes & Endocrinology in 2023 and covering more than 113,000 adults across 31 states and union territories, found that 11.4% of adults in India have diabetes and 15.3% have prediabetes. If your report has a bold line on it, you have a lot of company.

People of South Asian origin also tend to develop type 2 diabetes younger, and at a lower body weight, than people of European origin. Weight carried around the middle is a stronger warning sign than overall weight. This is why the national screening programme checks blood sugar in every adult from 30 years of age, not only in people who are visibly overweight.

Symptoms, in the words people actually use

Prediabetes almost never causes symptoms. That is why it is usually found on a routine test. When blood sugar climbs higher, people describe:

- "I am going to the bathroom all the time, and I get up at night to pass urine"

- "I am always thirsty, however much water I drink"

- "I feel tired all the time, even after sleeping"

- "My eyes go blurry on and off"

- "I have lost weight and I was not trying to"

- "Small cuts take ages to heal," or repeated boils, itching or fungal infections in the groin or under the breasts

If any of these sound familiar, do not wait for the next annual check. Book an appointment now.

🔴 When to go to a hospital, not a clinic

Very high blood sugar can become an emergency, sometimes within a day. Go to a hospital emergency department, or call 112, if you or someone with you has:

- Vomiting, stomach pain, and great thirst together, especially in someone known to have diabetes

- Deep or fast breathing, or breath that smells fruity or like nail polish remover

- Drowsiness, confusion, or is unusually hard to wake

- A home sugar reading that stays very high and comes with any of the above

- Signs of very low sugar in someone on diabetes treatment that do not settle after taking something sweet, or any low-sugar episode with confusion, a fit or loss of consciousness

- A foot wound that is red, hot, swollen, smelly or turning dark, with fever

- Chest pain, weakness on one side of the body, slurred speech or a drooping face. Diabetes raises the risk of heart attack and stroke, and both need a hospital, now

Do not drive yourself. Take a list or the strips of any medicines you take.

What actually helps

The most encouraging fact about prediabetes is that the evidence for lifestyle change is strong, and some of it comes from India. In the Indian Diabetes Prevention Programme, adults in Chennai with raised blood sugar were followed for three years. Among those given only routine advice, 55% developed diabetes. Among those supported to change their diet and activity, 39% did, a reduction in risk of about 28.5%.

None of it needs a special diet plan or a supplement:

- Walk briskly for 30 minutes, five days a week. About 150 minutes a week is the figure most guidelines use. Splitting it into a 10-minute walk after each meal works too, and it blunts the sugar rise after eating.

- Lose a little weight if you are carrying extra around the middle. In the international trials, losing around 5 to 7% of body weight was enough to make a real difference. For someone at 80 kg, that is 4 to 6 kg.

- Cut sugary drinks first. Sweetened tea, packaged juices, cold drinks and lassi with added sugar are the easiest calories to lose.

- Watch the size of the rice and roti portion, not only the sweets. Half the plate as vegetables or dal, a quarter as grains, is a simple rule that holds up.

- Stop smoking. It raises the risk of diabetes and of every complication diabetes brings.

About festive sweets. A piece of mithai at a celebration will not give you diabetes, and nobody needs to refuse every one. What matters is the pattern over weeks: the box opened every evening, the second and third piece, the sweet tea with each visit. "Sugar-free" sweets are often still made with flour, khoya and ghee, and can be almost as heavy as the ordinary kind. Eat less of either rather than more of the "free" one.

If you are diagnosed with diabetes, the doctor may suggest medicine as well as these changes. Do not start or stop anything on your own, and if you are unsure what a tablet is, bring the strip to the appointment.

What we will not do

- We will not label you diabetic from one borderline number. If your result is near a cut-off, we repeat it, or confirm it with a different test, before anything is written down.

- We will not put you on medicine for prediabetes as a reflex. For most people the first step is lifestyle change and a repeat test, usually within a year. If your numbers or your other risks suggest otherwise, the doctor will explain why.

- We will not sell you a 60-test health package. For blood sugar, a small number of tests answers the question. We will tell you which ones and why.

- We will not rely on HbA1c when it can mislead. Anaemia, thalassaemia trait, recent blood loss or transfusion, kidney disease and pregnancy can all push the HbA1c up or down. In these situations a fasting sugar or a glucose tolerance test is more reliable, and we will say so.

- We do not have an endocrinologist on our panel. Type 1 diabetes, diabetes in pregnancy, and diabetes that stays poorly controlled despite treatment are better looked after by a diabetes specialist. If that is what you need, we will tell you plainly and refer you.

Tests, and when they are worth doing

For most adults with a borderline result, the doctor will suggest some of these:

- HbA1c, to confirm or rule out the first result. It needs no fasting.

- Fasting blood sugar, especially when HbA1c may not be reliable.

- A lipid profile (cholesterol and triglycerides), because raised sugar and raised cholesterol often travel together, and together they shape your heart risk.

- Blood pressure and waist measurement, taken at the consultation.

- If diabetes is confirmed: a kidney function test and a urine test for protein, and a referral for an eye check of the retina by an eye specialist, since diabetes can damage the small blood vessels in both.

A glucose tolerance test is used when the picture is unclear, or in pregnancy.

What it costs

| | Fee |

|---|---|

| General physician consultation | from ₹800 |

| Senior family physician consultation | ₹1,200 |

| HbA1c | ₹399 |

| Lipid profile | ₹399 |

Fees are for booking directly with us. Fees for appointments booked through third-party platforms are set on those platforms and may differ. Blood samples can be collected at home free of charge, with no minimum order. For any other test the doctor suggests, we tell you the price before anything is booked.

Who you will see

A general physician is the right first doctor for a raised sugar result. Prediabetes and most type 2 diabetes are managed well in general practice, and a doctor who knows your blood pressure, weight, cholesterol and family history is better placed than a specialist seeing one number.

Dr. Waquar Yunus (MBBS), our general physician, is in the clinic for the full clinic day, Monday to Saturday, 8:00 AM to 9:00 PM, so you can be seen before or after work.

Dr. Pratik Banerjee (MBBS, PGCC, Paed (IMA)), family physician with 37 years' experience, consults from 11:00 AM to 1:30 PM and 5:00 to 7:00 PM. Dr. Sushma Vikirna (MBBS, PGDMCH), senior consultant with 40 years' experience, consults Monday to Saturday from 2:00 to 5:30 PM.

Bring every previous report you have. A trend over three years tells the doctor far more than one number.

Frequently asked questions

Is prediabetes the same as diabetes?

No. It means your sugar is above normal but below the diabetes range. Many people with prediabetes never develop diabetes, especially if they make changes early, and some return to the normal range.

Can my sugar go back to the normal range?

For many people, yes. Weight loss, regular activity and fewer sugary drinks bring many people's numbers back towards normal. We avoid promising it for any one person, because age, family history and other conditions all play a part, but the odds are better at this stage than at any later one.

Do I need to fast for an HbA1c test?

No. HbA1c reflects your average sugar over roughly the last three months, so the time of your last meal does not matter. A fasting blood sugar does need 8 hours without food; water is fine.

How often should I repeat the test?

With prediabetes, usually once a year, or sooner if you develop symptoms or the doctor advises it. With a normal result and no risk factors, every few years after 30 is reasonable.

Can I eat sweets during the festivals?

In small amounts, yes. The problem is the daily habit, not the single celebration. "Sugar-free" sweets are not a free pass: many are just as rich in fat and flour.

Can I come without an appointment?

Please call first and we will give you a time. If you have symptoms that worry you, say so on the phone and we will find you the earliest slot. With vomiting, drowsiness, confusion or any of the hospital signs above, go to a hospital emergency department instead.
