Ear Wax Removal by Microsuction: What Happens, How It Feels, and What It Costs

One ear feels blocked, your voice sounds odd inside your head, and the cotton bud only seems to make it worse. Someone suggests syringing. Someone else has heard of microsuction and asks whether it is safer.

Microsuction is a way of removing ear wax with a fine suction tube while the doctor looks directly into the ear under magnification. It uses no water. At All Is Well it is done by our ENT surgeon, at the clinic. This page explains what happens, how it feels, who it suits, and when your ear needs something other than wax removal.

Do you need your ears cleaned at all?

Usually not. Ear wax is not dirt. It traps dust, keeps the skin of the ear canal from drying out, and moves outward on its own, helped by chewing and talking. Most ears never need cleaning.

Wax becomes a problem when it builds up and blocks the canal. That is more likely if you:

- Use cotton buds, hairpins or keys, which push wax deeper

- Wear hearing aids or in-ear earphones for long hours

- Have narrow or hairy ear canals

- Are older, when wax tends to be drier and harder

- Have had blocked ears from wax before

Symptoms, in the words people actually use

- "My ear feels full, like after a flight"

- "I can hear my own voice echo"

- "Sounds are muffled on one side"

- "A ringing or buzzing in one ear"

- "My ear itches deep inside"

- "I feel slightly off balance"

These symptoms are not always wax. Fluid behind the eardrum, an infection, or a problem with the hearing nerve can feel exactly the same, which is why the ear is examined before anything is removed.

Microsuction, irrigation and manual removal

There are three established ways to remove wax. The right one depends on your ear, not on a preference for one method.

| Method | How it works | Points to know |

|---|---|---|

| Microsuction | A fine suction tube lifts the wax out while the doctor watches under magnification | No water, so it can be used when the eardrum has a hole or there are grommets. It is noisy |

| Irrigation | A controlled stream of body-temperature water flushes softened wax out | Not suitable with a hole in the eardrum, grommets, an infection, or past ear surgery |

| Manual removal | Small instruments lift the wax out under direct vision | Useful for firm wax near the entrance of the canal |

The UK's National Institute for Health and Care Excellence (NICE) recommends microsuction or electronic irrigation for wax that needs removing, and advises against the old hand-held metal syringes. There is no good-quality trial directly comparing microsuction with irrigation, so neither can honestly be called better for everyone.

The advantage of microsuction is that the doctor can see the wax, the canal and the eardrum the whole time, and no water goes into the ear. At the clinic, Dr. Mundra uses microsuction or manual removal, whichever suits your ear, and often both in one visit.

Before your appointment: soften the wax

Hard wax comes out more easily, and more comfortably, once it is softened. Unless the doctor has told you otherwise:

- Lie on your side with the blocked ear facing up

- Put 2 to 3 drops of plain olive oil, at room temperature, into the ear

- Stay lying down for 5 to 10 minutes

- Do this 3 to 4 times a day for 3 to 5 days before your appointment

Do not put anything in the ear if you have ever been told you have a hole in the eardrum, if you have grommets, or if the ear is painful or has any discharge. Tell us when you book.

In one study of patients having microsuction, those who had softened their wax beforehand had less pain and less dizziness during the procedure.

What happens during microsuction

1. The ear is examined first. The doctor looks into both ears to confirm that wax is the problem and that there is no infection.

2. You lie or sit still with your head turned to one side.

3. The suction tube is guided into the canal while the doctor watches through the magnifier. It does not need to touch the eardrum.

4. The wax is lifted out in pieces. Soft wax often takes only a few minutes per ear. Hard wax takes longer, and a firm piece near the entrance may be lifted out with a small instrument instead.

5. The canal and eardrum are checked again once they are clear.

If the wax is very hard and will not come away comfortably, the doctor will stop. You may be asked to keep softening it for a few more days and come back, rather than having it forced out in one sitting.

How it feels, and the side effects

Most people do not find microsuction painful. What they notice is the noise, a loud whooshing close to the ear, and sometimes a brief cool feeling.

In a study of 164 people having microsuction at an ENT clinic, about half reported some side effect. Almost all were minor and short-lived. The commonest were:

- Dizziness, usually lasting seconds to minutes, because suction cools the ear canal slightly

- The loudness of the procedure

- Hearing feeling dull for a short while afterwards

Less often, the skin of the canal is grazed and bleeds a little. If anything hurts during the procedure, say so and the doctor will stop.

## When it is not wax, and when to go to a hospital

Go to a hospital emergency department the same day if:

- You suddenly lose hearing in one ear over a few hours or days, with no obvious blockage. This needs an urgent hearing assessment by an ENT, because treatment is more likely to help when it starts early

- There is severe ear pain with high fever, or swelling, redness or pain behind the ear

- Half of your face has become weak or droops

- A child has pushed a button battery or a magnet into the ear, or anything is stuck in the ear after an injury

- There is bleeding or clear fluid from the ear after a head injury

See a doctor, not just for wax removal, if there is discharge from the ear, the ear keeps hurting, the ringing does not settle, or you feel dizzy for long spells.

What we will not do

- We will not remove wax that is not blocking your ear. A thin layer protects the canal, and taking it out leaves the skin dry and itchy.

- We will not remove wax without examining both ears first. If the problem turns out not to be wax, we will tell you that instead of charging for a procedure.

- We will not force out hard wax in one sitting if it is hurting. Softening it for a few more days is safer.

- We will not do microsuction at home. It needs the equipment at the clinic.

- We will not recommend ear candles or home "ear vacuum" kits. There is no evidence they remove wax, and candles can burn the ear.

## Keeping wax from building up again

- Stop using cotton buds inside the ear. Wash the outer ear with a cloth.

- If you get blocked ears often, a few drops of olive oil once or twice a week can keep wax soft. Ask the doctor first if you have had ear problems before.

- Hearing aid and earphone users: clean the devices regularly and give your ears breaks.

- Some people simply make more wax. If yours blocks every few months, an occasional check is reasonable. A fixed schedule of cleaning is not.

Who you will see

Dr. Aparaajita Mundra, our ENT surgeon: MBBS, MS (ENT) as a gold medallist, and DNB, with 10 years' experience. She is the only doctor here who removes ear wax, by microsuction or by hand with small instruments, at the clinic. She consults Monday to Saturday, from 2:30 to 4:00 PM. Specialist slots are limited, so please call for her next available time.

If you are not sure whether it is wax, Dr. Waquar Yunus (MBBS), our general physician, is in the clinic for the full clinic day, Monday to Saturday, 8:00 AM to 9:00 PM, from ₹800. He can look in the ear and tell you whether you need the ENT.

Frequently asked questions

Does microsuction hurt?

Most people feel no pain. It is noisy, and some people feel dizzy for a minute. Softening the wax for a few days beforehand makes it more comfortable. Tell the doctor straight away if anything hurts.

Can I have microsuction if I have a hole in my eardrum or grommets?

Usually yes. Because no water is used, microsuction is often the preferred method for these ears. Tell us about any ear surgery, hole or grommets when you book, and do not use oil drops beforehand.

How long does it take?

Often a few minutes per ear once the wax is soft. Hard or deep wax takes longer, and occasionally needs a second visit after more softening.

Can children have microsuction?

Children have to keep their head still and cope with the noise. Dr. Mundra will examine the ear and advise whether microsuction or another approach is better for your child.

Will my hearing come back straight away?

If wax was the only problem, hearing usually improves as soon as the canal is clear. If it does not, the doctor will look for another cause.

Can I come without an appointment?

Please call first and we will give you a time with the ENT. If you have suddenly lost hearing in one ear, do not wait for a wax appointment: go to a hospital the same day.
